Permanent, full time (37.5 hpw), working shifts (07:00 - 19:30 or 09:00 - 21:30), taking turns on covering weekends and bank holidays

We support our customers with range of support needs which may include frailty and dementia, we help them regain or maintain their independence and live well in their own homes. Some of our customers have more complex support needs and may require personal care, including bathing, toileting and getting dressed.
What you'll do
* We provide 24-hour support to our customers
* We have 82 apartments and our customers are 65+ with Dementia, or some care needs.
* Developing customer care plans, coordinating reablement workers to deliver on the plans
* Leading on caseloads, risk assessments, regular reviews
* Working collaboratively with external agencies
