Rotation Overview
* The Falls Prevention service is a multidisciplinary team providing community based assessments for service users who are at risk of falling. Therapists take a holistic approach to treatment following completion of a multifactorial assessment and provide rehabilitation either at home, in a clinic setting or in one of our group based exercise classes.
* The Domiciliary Physiotherapy Service supports therapists to manage a varied caseload ranging from the management of frailty to treating patients with orthopaedic and musculoskeletal conditions.
* The Intermediate care service works in partnership with Lancashire County Council within short term beds to ensure patients are safely discharged from hospital. This specialist team rehabilitates patients back to their base line, before working closely with the individual and their families to facilitate a timely return home.
