An exciting opportunity has arisen for a Band 5 Community nurse working within the Woodbridge Integrated Neighbourhood Team. We are a busy dynamic team who actively support staff new to the community. This position is being offered on a permanent basis at 37.5hours per week. Experience is preferred but not essential as full training will be given.
The motivated and forward-thinking team consists of Nurses, Occupational therapists, Physiotherapists, Social Workers and Support Staff. The team also work closely with other internal and external services or agencies.
The primary function of the Integrated Neighbourhood Team (INT) is to provide healthcare in the patient/customers home environment. You will be part of a multidisciplinary team providing interventions, such as short-term rehabilitation, wound care, and end of life care. The INT also supports with admission avoidance, timely discharge, and promotion of independence through holistic assessment and informed choice. In addition to the visiting patients/customers within their own environment, the team also have the use of a gym and clinic setting at Stow Lodge Centre. These spaces are used to run clinics, such as Falls Clinic, Better Balance group, Leg Ulcer clinic, catheter clinic, PICC Line Care.
The team work closely with 7 GP practices and there is a wealth of experience and knowledge within the current members of the team.
Main duties of the job
Your duties would include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multi-disciplinary team meetings, PICC line care, male, female suprapubic catheterisation, ordering equipment, support and mentoring of studentsand involvement in link roles such as tissue viability.
