We have a core background team and offer shifts that may include overnight support. This is an exciting opportunity to join Community Integrated Care at our supported living service in Hemsworth.


Responsibilities

* Provide empathic, patient, person-centered support to individuals living in our services.
* Encourage and assist people to work towards outcomes that enable independence.
* Support people to engage in the local community, including activities such as clubs, cinema, shows, days out, gym, shopping, and personal choice activities.
* Support a community within Wakefield across several services and help maintain relationships and friendships.
* Work with new partnerships and participate in community activities and events.
* Ensure a safe, inclusive environment and follow health and safety guidelines and company policies.
