Role Overview: As a Field Support Officer, you will play a vital role in supporting the operational delivery of Home First services. You will be responsible for conducting care and assessment visits in client's homes, mentoring and coaching colleagues, conducting audits and spot checks, and ensuring compliance with CQC regulations. You will be working closely with the Home First team, as well as external partners like Safeguarding, Hospital@Home, District Nursing, and others to ensure the delivery of high-quality care. Key Responsibilities: Support the Home First team in delivering high standards of care and support to clients in their homes. Conduct care and assessment visits, including the completion of care plans and risk assessments. Induct new staff, providing guidance, mentoring, and training to support their development. Conduct regular audits, spot checks, and observations to monitor the quality of care. Act as a key point of contact for colleagues and provide feedback and support for service improvements. Promote excellent care practices that respect individuality, values, and diversity. Support with crisis management and ensure service delivery is maintained during periods of high demand or staff sickness. Work with a variety of external partners to ensure a coordinated approach to client care. Skills & Experience Required: Essential Criteria: Minimum of 5 years of experience in health and social care, including 2 years within domiciliary care or reablement. Experience in safeguarding adults, writing care plans, conducting assessments and reviews. Knowledge of CQC regulations and compliance. Ability to work independently and adapt to changing priorities. Strong communication skills, both written and verbal. Experience with electronic care systems and a good understanding of technology in care settings. A Level 3 qualification in Health and Social Care (or working towards). Ability to travel within the county and hold a full UK driving licence. Desirable Criteria: Experience in delivering personal care in people’s homes. Knowledge of the Reablement sector. Ability to conduct audits and quality checks. Additional Information: This position requires an Enhanced DBS check. The role will involve working a rota across 7 days a week, with shifts from 07:00 - 22:00.
