We are working closely alongside a Local Authority in Southwark to assist with the appointment of a Independent Living facilitator, on a 2.5-month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Rate of Pay: £17.26- £22.77 per hour

Summary:
The Independent Living Facilitator works in the Independent Living Team, based at the Southwark Resource Centre. The role involves working flexibly with adults who have Physical Disabilities, Learning Disabilities, Sensory needs, a range of these, or are older adults with Dementia. The purpose is to provide outcome-focused, strength-based, and time-limited interventions to maximise opportunities, social inclusion, wellbeing, and promote independence and resilience. The post contributes to delivering a programme of respite, reablement, and social inclusion services for eligible clients.
Responsibilities:
* Provide outcome-focused, strength-based, and time-limited interventions for adults in the community, their own homes, and at the Southwark Resource Centre
* Facilitate and deliver group and individual activities as part of a clients agreed support plan.
* Provide personal care support and assistance with moving and handling needs as identified in the tailored support plan.
* Ensure all GDPR principles are followed
* Maintain accurate service and client records on a regular basis using paper and electronic systems, and contribute to progress reports and reviews
Essentials:
* NVQ level 3 in Health and Social Care
* Experience of working with adults with physical disabilities either in a paid or voluntary capacity and providing tailored support
* Experience of developing outcome-focused, person-centred support planning with adults with physical disabilities; and assisting individuals with support planning or goal setting

* This role requires an Enhanced DBS
