Kingston Health is a busy teaching practice serving 10,000 patients. We are looking to recruit an Experienced Care Navigator, Part-time. This is an exciting opportunity to join a well-established and high-performing practice to improve the service we offer to patients whilst supporting the GPs.
The candidates we seek should be experienced in delivering excellent reception support, maintaining professionalism and discretion, and able to work independently and as part of a team. Good communication skills and attention to detail are essential, along with flexibility to cover for annual/sick leave. Knowledge of SystmOne would be advantageous.
You will have experience working with the public, possess a cheerful and polite manner, be capable of multi-tasking, and be a good team player. Maintaining patient confidentiality, both inside and outside the practice, is mandatory.
Main duties of the job
You need to be a good communicator with excellent interpersonal and customer service skills, especially under pressure. Your role will involve a range of patient-facing and administrative tasks, requiring excellent organisational skills and time management. A caring, empathetic character is essential.
Your duties will include answering telephone calls, registering patients, and general admin within the practice. A 'can do' attitude and flexibility to cover shifts when needed are important.

Kingston Health is a teaching practice with just over 10,000 patients, including 4 GP partners, 1 Physician Associate, 2 nurses, 2 HCAs, 1 Pharmacist, care navigators, and medical secretaries based across two sites: Wheeler Street and Holderness Road.
Job responsibilities
* Greet patients and visitors to the practice.
* Book in patients and visitors according to practice appointments and visitor procedures.
* Respond to or redirect all patient and visitor requests, ensuring unresolved and urgent matters are passed to colleagues at the end of shift.
* Open incoming mail and distribute accordingly.
* Book, amend, and cancel patient appointments.
* Record patient information onto the clinical system.
* Answer incoming calls, document, and redirect them appropriately.
* Care navigate all patients, including referring patients to outside providers.
* Be flexible to provide holiday and sickness cover regularly.
* Maintain both computerized and manual filing systems (scanning, filing, etc.).
* Work safely at all times according to legislative requirements and practice policies.
* Handle administrative duties such as registrations, updating new patient questionnaires, and deductions.
* Perform any other duties as required; this job description may be adjusted periodically.
Person Specification
Qualifications
* GCSE Grade A-C or equivalent.
* NVQ Level 2 or above in Customer Service.
* Excellent written and oral communication skills.
* Good time management and ability to meet deadlines.
* Negotiation and conflict management skills.

* Knowledge of System One.
