As a Nurse with TLC Care, you are responsible for achieving the highest possible standard of care in a professional manner through direct nursing care and effective supervision of employees. You would be expected to provide leadership to the care team, and assess, develop, and evaluate individualised resident care, ensuring consultation with and involvement from residents, GPs, relatives, and carers.

Main Purpose
To achieve the highest possible standard of care in a professional manner through direct nursing care and effective supervision of employees.
To take management responsibility for a shift as directed.
Main Responsibilities
Care and Care Planning:
* Report any ill health amongst residents and make requests for GP/Professional visits where necessary.
* Ensure that resident’s Care Plans are completed and maintained in conjunction with residents, relatives, and other healthcare professionals, in accordance with NMC guidelines & company policy.
* Assess all aspects of residents’ care needs and provide health supervision and direct nursing care when required.
* Assess social and healthcare needs of new residents and maintain ongoing assessment and review of all other residents.
* Maintain accurate records and ensure that each resident within the Care Centre has an individualised Care Plan.
* Administer prescribed medicines and document the same in accordance with NMC guidelines.
* Be the named Nurse for a number of residents.
Tissue Viability:
* Ensure there are no home-acquired pressure sores by delivering a high standard of care.
* Report and detail any pressure sores to appropriate parties.
* Ensure residents at risk of pressure sores have an individualised care plan.
Nutrition:
* Ensure residents have a MUST assessment completed monthly.
* Ensure that residents at risk have an associated care plan, detailing interventions required.
* Ensure fluid and food intake charts are in place for residents with nutrition or hydration problems.
* Record special dietary requirements in the care plan and communicate this to the kitchen.
* Ensure residents requiring food via PEG have a care plan in place.
Continence:
* Complete continence assessments where required.
* Ensure all residents with catheters have a care plan detailing catheter care.
Infection Control:
* Uphold and promote the home’s infection control policy.
* Monitor staff to ensure they maintain universal precautions, wash hands appropriately, and wear gloves and disposable aprons when dealing with body waste.
Resident Hygiene:
* Ensure residents are well groomed, wearing appropriate clothing, and that care records detail resident choices.
Resident Mobility:
* Enable residents to move independently when possible and ensure walking aids are accessible.
* Check that bedrails are not used routinely.
* Residents at risk of falling must have a care plan with interventions to minimize risk.
Dignity and Respect:
* Address residents by their preferred name in a courteous manner.
* Ensure care staff knock on residents’ doors and engage respectfully.
Communication:
* Participate in staff, resident, and relative meetings as required.
* Maintain good communication with residents, relatives, and the multidisciplinary team.
* Handle complaints effectively.
Training & Development:
* Act as a mentor to student nurses and carers.
* Maintain and improve professional knowledge and competence.
* Attend mandatory training days/courses as required.
Health & Safety:
* Understand and implement the Care Home’s health and safety policies and emergency procedures.
* Promote safe working practices within the Care Home.
General:
* Adhere to all relevant NMC guidelines/regulations.
* Maintain records as required by authorities and management.
Skills, Knowledge, and Qualifications:
* NMC Registered Nurse – valid NMC PIN.
* Ability to motivate and delegate effectively.
* Effective communication skills at all levels.
* Genuine interest in working with the elderly.
* Satisfactory DBS and DBS Adult First check.
* Team player with ability to work across a multidisciplinary team.
