Overview

Are you enthusiastic, motivated and looking for a new and exciting challenge? Do you want to develop and extend your skills and knowledge within a Discharge Liaison Team? If so then this could be the job for you! York and Scarborough Teaching Hospitals NHS Foundation Trust is looking to recruit an enthusiastic and self‑motivated Discharge Liaison Officer. You will be part of the Patient Flow team, working across the acute medical, surgical and elderly wards to optimise discharges and support patients and staff to negotiate discharge plans. The Discharge Liaison Officer works closely with ward staff, Social Care, Continuing Health Care and the voluntary sector. Therefore you must be able to demonstrate collaborative working, as well as building effective relationships with colleagues from other healthcare professions. This role entails working closely with a wide multidisciplinary team to facilitate the timely discharge of patients from the hospital environment.

Requirements

Expect to join a weekend rota. The ability to escal­e concerns via appropriate channels of escalation is essential. The successful candidate will be required to use a variety of computer applications and should be confident in IT skills.
