Job summary

Chief Registrar in Intensive Care Medicine

You must be ST5 or above (or equivalent) to be eligible for this role.

Start date: September 2026

Contract type: Fixed term (12 months)

The chief registrar role is a leadership role for senior specialty residents. The role provides 25% protected time to develop and implement local initiatives focusing on service improvement, engagement and morale, education and training, workforce and sustainability. This role is a Chief Registrar in Intensive Care Medicine and is open to those training in Intensive Care Medicine in stage 2 or 3 as part of their specialty training or equivalent portfolio pathway.

Chief registrars benefit from access to a bespoke development programme provided by the RCP, which runs from September to June and comprises five 2-day modules that chief registrars are expected to attend.

The chief registrar will report to and be mentored by the service lead, clinical director and medical director. The chief registrar will also have a named educational supervisor for their role, who may or may not be the same as their clinical supervisor.

There will be formal educational oversight of the role, with an induction, educational agreement, personal development plan and regular appraisals.


Main duties

The chief registrar job description is broad in scope to allow chief registrars and recruiting organisations to have autonomy and flexibility over the work the chief registrar undertakes. Chief registrars should focus on addressing key local challenges and priorities, which may include some or all the following:

* Providing a 'bridge' between senior clinical leaders, managers and the wider trainee workforce to improve communication, engagement and morale.
* Service improvement, for example redesigning pathways, implementing new technology and establishing new services to improve flow and outcomes for patients.
* Improving the quality of clinical and non-clinical education and training activities and supporting/mentoring other trainees to engage in quality improvement.
* Involvement in workforce planning and improving the deployment of trainees to meet service needs and improve morale.
* Improving efficiency and reducing waste.
* Working across teams and boundaries to engage stakeholders in quality improvement and influence change.
* Involvement in and exposure to senior management and organisational decision-making

The chief registrar will attend departmental and divisional management meetings to gain an understanding of management and the wider social, political and economic influences on healthcare delivery.


About us

The Critical Care department at GSTT has an international reputation for clinical excellence and a prominent research profile. As well as serving the local population we provide tertiary critical care to the region. The units are equipped with state-of-the-art monitoring, point of care testing and a computer information system for charting, note-keeping and prescribing. All modalities of organ support are available.

The critical care department comprises six clinical areas and totals 85 beds across the St Thomas' and Guy's sites. There are 41 level 3 beds in the East Wing of the hospital (15 on EW1 ICU, 15 on EW2 ICU and 11 on EW6 ICU), an overnight intensive recovery unit (11 level 3 beds staffed by anaesthetists), a medical/surgical HDU on EW10 (15 level 2 beds) and cardiovascular surgical HDU (10 level 2 beds) on EW8. At the Guy's site, there are 13 level 2/3 beds on the Guys Critical Care Unit (GCCU).

Our case mix is challenging with a large proportion of complex medical and cardiothoracic patients. We manage a wide spectrum of medical and surgical emergencies and take patients from the emergency department, wards, theatres and external referrals from other Trusts.

We are one of the largest severe respiratory failure centres in Europe by case load with a 24/7 consultant-led ECMO/severe respiratory failure retrieval service. The post holder will also have significant exposure to general ITU patients and complex vascular, general surgery and cardiac surgery patients.

Job responsibilities

In addition to the chief registrar duties above, the post holder will have the following responsibilities:

Clinical:

The postholder will, together with colleagues, be responsible for the provision of services to the Guys & St Thomas NHS Foundation Trust to include:(a) Diagnosis and treatment of patients of the trust in such hospitals, health centres or clinics or other premises as required.(b) Continuing clinical responsibility for the patients in your charge, allowing for all proper delegation to, and training of, your staff.

Training of junior staff:

The postholder will take responsibility for the training and direction of junior staff allocated to him/her under aegis of the training plan that that postholder has agreed with their supervising consultant.

Teaching:

The postholder will be expected to contribute as appropriate in the teaching of undergraduate and postgraduate students.

Clinical Governance:

All medical and dental staff are expected to take part in clinical governance activity, including clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their appraisal.

Mutual Obligation to Monitor Hours

There is a contractual obligation on employers to monitor working hours through robust local monitoring arrangements supported by national guidance, and on individual doctors to co-operate with those monitoring arrangements.

European Working Time Directive (EWTD):

All posts and working patterns are under constant review in line with EWTD guidelines

Appraisal:

All medical and dental staff are required to undertake appraisal.

Study Leave:

Study Leave will be granted at the discretion of the Clinical Lead.

Funding for CPD will be equivalent to that of the appropriate Training post.

Qualifications and experience

Essential

* Evidence of satisfactory / more than satisfactory progress through training, including annual review of competence progression (ARCP) outcomes

Desirable

* Additional relevant degree (intercalated, Masters or doctorate)

Clinical skills

* Demonstrates safe, effective and independent clinical practice appropriate to specialty and stage of training, with sound judgement, prioritisation of need and a strong focus on patient safety and risk management.

Research

* Demonstrates understanding of research principles and methodology, ethical considerations, and evidence-based practice, with evidence of academic achievement such as publications or presentations.

Leadership and management

* Demonstrates effective leadership and team working, self-awareness, and engagement with management systems, with an understanding of the local and national NHS context and evidence of leading improvement or innovation.

Quality / service improvement / audit

* Demonstrates understanding of clinical governance and quality improvement, with evidence of active involvement in audit or service improvement activity that improves patient safety and quality of care.

Education and training

* Demonstrates commitment to medical education through teaching experience, positive feedback, and engagement in teaching development or formal education and training activities.

Personal skills

* Demonstrates excellent communication, organisation, problem-solving, resilience and professionalism, with the ability to work autonomously and collaboratively while maintaining integrity, adaptability and commitment to ongoing development.
