Typical day as a Mental Health Lead Support Worker (known by us as Support Coordinator)
1. Supporting working age adults with primary mental health needs and secondary needs of substance misuse, physical disability, learning disability and autism.
2. You will start your shift with a handover ensuring you have up to date information about our customers and any changes in presentation, risk etc.
3. You will manage your own diary and be responsible for completing support sessions focused on things like making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation when they are able.
4. Carrying out risk assessments, support planning, goal setting and regular reviews.
