We have an opportunity for a Senior Occupational Therapist to join our supportive, diverse and fun team at Gosport War Memorial Hospital inpatient rehabilitation wards.
Responsibilities
* Provide occupational therapy consultations to patients referred from the acute trust and community teams.
* Step up vulnerable patients to prevent hospital admission.
* Deliver ward-based therapy and complete visits to patients' homes to facilitate discharge plans and continuity of care.
* Collaborate with the multi-disciplinary team to promote independence and improve quality of life.
We take a range of referrals, predominantly from the acute trust and community teams; stepping up vulnerable patients to prevent hospital admission. Our patients come with a variety of needs, from simple to complex and with a range of conditions and co-morbidities ranging from orthopaedic and neurological to ICU step downs, giving you the opportunity to think outside the box and enhance knowledge and skills.
The Occupational Therapy Team pride themselves on their ability to innovate and adapt to the challenges of rehabilitation with active quality improvement programmes, to ensure that practice is current, and the patient voice is heard.
Our vision is providing Quality Care which uses a person-centred approach, working closely with a wider multi-disciplinary team to achieve exceptional patient care and outcomes.
The Team consists of Occupational Therapists, Physiotherapists, Nurses, Doctors, Consultants, Social Workers and Healthcare Assistants who work together in an integrated way to provide co‑ordinated, clinical support to promote independence and to improve quality of life.
