<div><h3>EAS - Care Coordinator -18.75 Hours - Hyndburn Central</h3><p>The East Lancashire Alliance is recruiting a care coordinator to join the team within the Hyndburn Central PCN.</p><p>This role will involve working with the various practice sites across Hyndburn Central, and may interlink with other teams such as the Pharmacists. Initially a fixed‑term contract of 2 years, with the possibility of extension or becoming permanent. The post holder must be flexible regarding the working pattern to ensure all practices within the PCN are supported according to patient needs.</p><h3>Main duties of the job</h3><p>The post holder will be responsible for the provision of wide‑ranging and efficient administrative support to the Hyndburn Central Primary Care Network (PCN). Duties typically include arranging meetings, handling correspondence, managing documents, preparing short reports, creating spreadsheets, responding to and forwarding e‑mails, and taking minutes of PCN meetings.</p><h3>Job responsibilities</h3><ul><li>Support the Care Coordinator team within the PCN Administration Team, coordinating care for patients registered with practices and care homes.</li><li>Work closely with GPs and other primary care professionals to support delivery of quality patient services.</li><li>Act as a key point of contact for residents, families and professionals to improve continuity of care.</li><li>Develop and implement data collection systems to monitor and evaluate services.</li><li>Provide a first point of contact for patients and clinicians in coordinating care.</li><li>Support the Enhanced Access service, including organising staffing rotas, completing patient referrals and day‑to‑day provision.</li><li>Respond to queries from patients, carers and healthcare providers.</li><li>Utilise GP Practice clinical systems (EMIS) and population health data to proactively identify patient cohorts for personalised care.</li><li>Encourage feedback and engagement from patients, families and carers.</li><li>Support Care Homes with vaccinations and other healthcare services.</li><li>Identify patient service needs or inequalities and coordinate projects with the Federation team and PCN CDs.</li><li>Support the coordination and delivery of multidisciplinary teams within the PCN.</li><li>Assist the PCN Transformation and Development Manager with administrative duties for other PCN activities.</li><li>Aid the PCN Clinical Director in collecting information, identifying anomalies and summarising data.</li><li>Gather information and undertake enquiries for the PCN and the wider group as required.</li><li>Collaborate with Integrated Neighbourhood Teams on identified projects.</li><li>Monitor tasks to ensure completion and delivery of care through regular audits.</li><li>Coordinate and navigate across health and care services with social prescribing link workers and other professionals.</li><li>Signpost and organise appointments, follow‑ups or other actions to help deliver high quality, compassionate care.</li></ul><h3>Person Specification</h3><h3>Qualifications</h3><ul><li>Demonstrable commitment to professional and personal development.</li><li>Proficient in MS Office and web‑based services.</li><li>NVQ Level 3 or equivalent, or relevant basic/first‑level professional qualification.</li></ul><h3>Experience</h3><ul><li>Minimum of 1 year of experience working with healthcare professionals, or previous experience in the NHS, social care, or a relevant field.</li><li>Excellent interpersonal skills.</li><li>Excellent organisational and administration skills.</li><li>Experience handling confidential/sensitive information.</li><li>Experience providing advice/signposting to service users.</li></ul><h3>Skills/Abilities</h3><ul><li>Able to prioritise and manage own workload.</li><li>Able to work as part of a team.</li><li>Experience coordinating and liaising with multiple stakeholders to meet specified outcomes.</li><li>Access to own vehicle and a clean driving licence.</li><li>Experience in coproduction with patients or service‑users.</li><li>Experience using technology and digital tools to support health and wellbeing.</li><li>Knowledge of Information Governance and data quality.</li><li>Knowledge of medical patient systems.</li><li>Understanding of health and social care processes.</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a Submission to the Disclosure and Barring Service (formerly CRB) to check for any previous criminal convictions.</p></div>
