Job Summary
Newport Pagnell Medical Centre are looking for an experienced care coordinator to join our Community Health and Support team on a part-time basis for 18 hours per week. The Community Health and Support team is part of our community teams and wider primary impact team under The Bridge PCN.
Main duties of the job
The Aim Of The Team Is To Support Patients And Their Carers With Health And Social Care Needs To Remain Living Well At Home To:
improve health and wellbeing
prevent multiple unplanned hospital admissions
reduce frequent use of GP services
proactively identify unmet needs in the community

Newport Pagnell Medical Centre is based in the town of Newport Pagnell and we have further sites within the Kingfisher area and Willen areas.We are a large training practice working across the three sites and we have our own PCN - The Bridge. Our ethos is compassionate care.
