Overview
We have an exciting opportunity for a rehabilitation‑focused Senior Occupational Therapist to join our Inpatient Rehabilitation Service on Welney Ward, Princess of Wales Hospital, Ely.
This position involves working with patients on our inpatient wards to support patients out of hospital, providing holistic clinical assessment, rehabilitation, discharge planning and specialist therapy knowledge.
Successful applicants will play a vital role in supporting patient flow through the pathways, providing clinical skills and knowledge to the MDT, and contributing to the transformation of our inpatient rehabilitation service. You will be supported by a Band 7 clinical lead.
Responsibilities
* Work as an autonomous practitioner within the integrated multi‑professional team.
* Be responsible for own caseload and undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
* Identify patient needs, agree goals and provide appropriate therapy interventions, provide adaptive equipment, rehabilitation programmes, and make recommendations for minor adaptations in line with current legislation.
* Keep the patient at the centre of care, ensuring patient and carer participation in decision‑making; provide support and education to peers, new staff, non‑registered staff and students.
* Be accountable for a delegated caseload held by Band 5 Therapists and Assistant Practitioners.
* Prioritise all patients’ tasks according to clinical need and allocate or signpost appropriately, providing a high standard of care.
* Work in various community settings within CPFT as required, completing access and home visits.
* Be professionally and legally accountable for all aspects of own and delegated work.
* Work within codes of practice and professional guidelines.
* Undertake holistic assessment (including those with complex presentations and multi‑pathologies) using specialist clinical reasoning skills.
* Use evidence‑based practice to ensure clinical care is effective and appropriate.
* Promote independence and, wherever possible, avoid acute hospital admission and reduce inpatient length of stay to ensure patients receive the appropriate care in the most appropriate setting.
* Organise and manage own time, delegating work appropriately.
* Conduct risk assessments and health and safety assessments, reporting any changes or newly identified risks.
* Demonstrate the importance of gaining patient and carer consent in all interventions.
* Effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills, including with patients who have difficulties in communication.
* Actively manage positive relationships with other agencies involved in the patient’s pathway.
* Actively involve in the development of the service and organisational aims and objectives, and the new model of care.
* Work effectively within the structure, liaising appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews and family meetings.
* Ensure that accurate and timely written records are kept in compliance with Trust policy and HCPC guidance, reporting on any issues as appropriate.
* Provide specialist advice to all members of the multi‑disciplinary team, service users and appropriate others regarding changes involving current care plans and patient progress.
* Work within Trust and COT guidelines, having a good working knowledge of national and local standards of practice in line with legal and operational requirements.
* Demonstrate a good understanding of Social Service and Health legislation.
* Support with day‑to‑day planning and delivery of high‑quality care.
