The post holder will work as an integral member of the Pharmacy Virtual Ward Team within South Tees Hospitals NHS Foundation Trust, promoting the safe, rational, effective, and cost‑effective use of medicines for patients managed within the Virtual Ward.
Responsibilities
* Perform medicines reconciliation and provide safe, therapeutic interventions for virtual ward patients.
* Offer patient counselling and clear medication information to patients and their families.
* Deliver pharmaceutical advice to multidisciplinary team members and support medication supply within the virtual ward.
* Analyse and interpret pharmaceutical information to contribute to multidisciplinary team discussions and virtual ward rounds.
* Provide training and education to multidisciplinary team colleagues on safe medicine use and medicines optimisation.
* Develop, implement, and evaluate a patient‑centred pharmacy service to support medication supply and pharmaceutical care for patients in their own homes, using technology and home visits.
* Ensure patient safety in relation to medicines for patients managed in their own homes as part of the virtual ward.
