Support Worker Job Description
Support Worker role for Citta Care Ltd in Canterbury. This is a hands-on support role where you will lead from the front.
The post is full or part time rostered over seven days on a permanent contract with a salary from £ £12.50 per hour depending on qualifications and experience. You will be expected to work shifts over a 24-hour period including sleep nights. As Citta Care is supported living service you will be expected to work weekends and bank holidays as part of your working week.

Support Worker Requirements
* Passes in English and Maths at GCSE Grade 4+ / C or a Functional Skills qualification to the equivalent
* Be able to complete the Care Certificate with the view to go on to complete at least an NVQ Level 3 in Health & Social Care following successful completion of probation
* Hold a full UK driving license with no more than 6 points and to have access to their own vehicle is desired but not essential
* Proof of valid MOT, road tax and insurance including business cover

Support worker Key Tasks
* Responsible for supporting our residents to become self-reliant and to develop their skills towards independent living
* Empower our residents to move on from our service after their two-year tenancy has expired, some tenancies may be extended depending on level of need
* Support residents to enable them to live a fulfilled and independent life in the community.
* Strike a good rapport with our residents, in order to support them to reach their full potential and achieve outcomes.
* Support our residents to appointments, travelling, socialising, completing forms and accessing community-based activities.
* You will be required to assist in the completion of daily reports, support plans and risk. This list is not exhaustive.
* Liaise with other agencies that are also supporting residents to ensure the best outcomes
* Become a key worker for a small cluster of residents taking responsibility for completing progress reports this includes meeting with the Service Manager and Team Leaders to discuss progress reports and any issues with the addition of reviewing documentation quality and support being provided.
* Participate in supervision and appraisals
* Ensure that work area is clean, tidy and safe, this is to include communal resident areas
* Keep up to date with any changes in policy and legislation and action accordingly
* Report any incidents/ concerns to your manager
* To carry out any other duties in accordance with this role profile.
