An exciting opportunity has arisen within the Ipswich East Community Team. We are looking for a band 5 Physiotherapist/Occupational Therapist who is passionate, committed, innovative, enthusiastic and highly motivated to deliver high quality care within patient homes.
The successful candidate will work co‑located with nursing, social care colleagues and the out‑of‑hours REACT, admission prevention and discharge assessment teams. The role is part of a knowledgeable, experienced multi‑disciplinary team that values flexibility, motivation and the ability to work in a changing environment. Support will be provided through in‑service training, professional forums, mandatory training, 1:1 and personal development meetings, and clinical rolling skills training, with close working relationships with neighbouring community teams.
Responsibilities
* Manage a defined caseload under the supervision of a Senior Clinician using evidence‑based, client‑centred principles to assess, plan, implement and evaluate interventions across local health care team settings, including inpatient, day and treatment, and community environments.
* Establish, maintain and participate in robust multi‑disciplinary communication networks with clients, carers, other health workers and agencies involved in client care.
* Promote the role of Occupational Therapy/Physiotherapy within the team, informing the Integrated Services Implementation Plan and negotiating priorities where appropriate, using high‑level presentation and training skills.
* Promote the role of multi‑disciplinary rehabilitation services within Suffolk Community Healthcare, informing the Integrated Services Implementation Plan.
* Participate in and maintain robust professional communication systems within own and other professional teams.
