Key Roles and Responsibilities:
1. Deliver high-quality, person-centred support tailored to individual needs and aspirations.
2. Utilise case management systems to review, monitor, and evaluate progress.
3. Facilitate key work sessions, reviews, and meetings in line with support plans.
4. Provide advocacy, advice, and support to ensure service users meet occupancy conditions.
5. Work in partnership with external agencies to maximise opportunities for supported individuals.
6. Manage a personal caseload while contributing to the wider team.
