Qualified Social Worker, Hospital Discharge and Post Discharge Assessment Team
Pay rate £37 per hour
Vitalis are recruiting for an experienced Social Worker in Calderdale Council. This is to work in a Hospital Discharge and Post Discharge Assessment Team. The hospital team covers Huddersfield Royal Infirmary and Calderdale Royal hospital, completing hospital discharge assessments. Cover may be needed in the post discharge assessment team if needed, who assess and review those people discharged from hospital into the community, whether they went home or into a care home placement. 37 hours a week.
