Overview
We are looking for Band 5 nurses to work within our district nursing teams based in Knowsley, to cover our 8am-8pm 7 days a week model, with fixed term vacancies available across the borough. The service is undergoing significant developments as part of our plan to deliver integrated community care for patients on a neighbourhood footprint across Knowsley.
The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model. Access to a private vehicle is required for this role - being able to travel between multiple sites in the course of performing duties is essential. The role will involve regular travel across healthcare organisations sites and patient living environments in the area of Knowsley.
Responsibilities
 * The post holder is responsible for the delivery of clinical care to a defined population. Supported by senior practitioners, they will deliver care within the boundaries of their role.
 * Work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership.
 * Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
 * Deliver care within the context of the service model (8am-8pm, 7 days a week) and adapt to the needs of patients across Knowsley.
 * Travel between multiple sites and patient living environments; meet travel requirements as part of the role.
Qualifications
 * Band 5 nurses; registered with the appropriate professional body.
 * Working to Mersey Care values with excellent empathy, communication skills and a person-centred approach.
 * Willingness to continue learning and develop competencies to provide evidence-based care to patients in their own homes.
 * Clinical decision-making skills and a problem-solving approach are essential.
 * Ability to work well as part of a team and autonomously, with the ability to escalate and report back appropriately.
 * Access to a private vehicle is required to travel between sites and patient environments across Knowsley.
