The advertised professional positions are part of our investment in and development of the Reablement service. This role involves supporting adults recovering from illness or trauma to regain independence through meaningful activities. Customers are referred from hospitals or their homes via professionals within the city. The service is fast-paced, with a time limit of up to 6 weeks, and often involves frail individuals with multiple health conditions, including physical and cognitive impairments. The care team collaborates closely with multidisciplinary team members including social workers, therapists, community nurses, and the community stroke team.
The Support Worker role is central to delivering excellent customer care and involves:
* Supporting to maintain or improve customer wellbeing.
* Providing person-centred care to vulnerable individuals with complex social and health needs.
* Supporting individuals to achieve maximum independence through physical, emotional, and psychological support, including intimate personal care, basic health support, practical tasks, communication, and community involvement. This includes following therapy plans and using innovative technology, for which training is provided.
