About the Role
We are seeking a warm, enthusiastic, and dedicated individual to join our award-winning team as a Team Leader. The role involves supporting the home management team to deliver high-quality, person-centred care in a safe, caring, and inclusive environment for residents, visitors, and staff.
Responsibilities include:
1. Leading the person-centred care planning process, including implementing, monitoring, evaluating, and auditing care plans, developing action plans, and ensuring their completion.
2. Assisting with the smooth transition of discharges, including documentation, medication management, and communication with residents' next of kin.
3. Monitoring residents' well-being, noting any changes impacting care, and updating care plans accordingly.
4. Practicing safe systems of work, assessing risks, and ensuring safety for residents, visitors, and staff during moving and handling tasks.
About You
To succeed, you should embody our values of caring, supportiveness, honesty, respect, and accountability.
Ideal candidates will have:
* An NVQ Level 3 or equivalent in care for care homes.
* At least two years' experience in a senior care role.
* Effective verbal and written communication skills.
* The ability to lead staff and act responsibly in the absence of the manager.
