Role Summary
You be responsible for delivering high‑quality, person‑centred care to individuals in their own homes. The role ensures that people receiving support are treated with dignity, respect, and compassion, and that their independence is promoted at all times. Driving is essential due to travel between client homes across the community.
Key Duties and Responsibilities
Personal Care and Support
Provide all aspects of personal care including washing, bathing, dressing, continence care, grooming, and mobility assistance.
Support with safe use of equipment such as hoists, slide sheets, mobility aids, and profiling beds in line with training.
Encourage independence while providing appropriate levels of assistance according to the individual’s care plan.
Maintain skin integrity, follow pressure area care procedures, and report any changes promptly.
Daily Living Support
Support with meal preparation, feeding, and hydration needs as specified in care plans.
Assist with medication prompting or administration (where appropriately trained and authorised).
Support with household tasks necessary to maintain a safe and comfortable environment.
Health and Well‑being
Monitor and report any concerns regarding health, changes in behaviour, or deterioration to the office immediately.
Follow all risk assessments, care plans, and clinical guidelines relevant to the individual.
Promote emotional well‑being through positive engagement and reassurance.
Travel and Scheduling
Use personal vehicle to travel safely and punctually between client visits.
Maintain accurate mileage records where required.
Adhere to allocated rota times and communicate scheduling issues promptly.
Record Keeping and Communication
Use Log my Care to record accurate, timely, and factual notes at each visit.
Report incidents, concerns, or safeguarding issues without delay.
Communicate professionally with clients, families, colleagues, and external professionals.
Essential Requirements
* Full UK driving licence.
* Access to a reliable, roadworthy vehicle suitable for community care work.
* Right to work in the UK.
* Good spoken and written English.
* Ability to follow care plans, clinical instructions, and organisational procedures.
* Reliability, punctuality, and strong organisational skills.
* Ability to work independently and as part of a team.
* Compassionate, respectful, and professional attitude at all times.
Desirable Requirements
* Experience in domiciliary care or health and social care.
* NVQ Level 2 or 3 in Health and Social Care (or willingness to work towards it).
* Experience supporting individuals with complex care needs, autism, or learning disabilities.
