Job summary
A new exciting opportunity has arisen for a Streaming Clinician with experience of working in emergency/urgent care settings, to join our team at OneMedical Group, and work for our Leeds urgent care LGI Minor Illness Service. This is a new role due to the growth of the service and an enhanced model of care.
Main duties of the job
As part of the day-to -day operation, the post holder will effectively stream patients that walk into the Emergency Department (ED) at LGI with undifferentiated conditions to the right clinical team that are able to manage the patients clinical presentation, for example, Minor illness service, ED department, SDEC or another speciality.

Job responsibilities
Usingthe VET (Visualise, Explore, Triage) model, the Streamer will be competent in recognising high risk presentations of all age ranges,both adults and children. The post holderwill notdischarge any patients from the ED.
The Streamer will use the hospital IT system, Symphony, to input the assessment information but also have use of SystemOne so that they are linked in with their clinical colleagues who work within the minor illness service.
They will have a permanent base, but there will be the expectation, on occasions, to work at one of our other OMG Urgent care sites.

LGI (Leeds General Infirmary) ispart of Leeds Teaching Hospitals Trust. The service is located either in or near to Leeds City Centre, has good public transport links and onsite parking.

Qualifications
Essential
17. First Level registered Nurse, Paramedic/Pharmacist.
18. Level 7 qualification in minor illness and/or clinical assessment and management skills
Desirable
19. Teaching qualification, 998 or equivalent
Experience
Essential
20. Experience of managing acute undifferentiated presentations
21. Experience of working in emergency/urgent care settings
22. Ability to triage/stream in fast pace environment
23. Experience in using based patient documentation systems.
Skills and Knowledge
Essential
24. Ability to manage and prioritise workload.
25. Good interpersonal and communication skills.
26. Good understanding of autonomous practice within a minor illness/urgent care centre and the roles that deliver this
27. Proven ability to work as part of a team.
28. In-depth Knowledge of the CQC Key lines of Enquiry
29. Current knowledge of government targets and initiatives and guidelines and their impact on clinical practice.
30. Knowledge of IT systems including System One, Symphony (desirable)
