Role Overview
An exciting opportunity has arisen for an Assistant Practitioner to join our Community Rehabilitation Team in Levenmouth. The Community Rehabilitation Team provides a service to the adult population within the various localities, delivering therapeutic treatment programmes in patients own homes to prevent hospital admissions and support early discharge.
Responsibilities
* Work with Occupational Therapy and Physiotherapy staff to deliver therapeutic treatment programmes to service users in their own homes.
* Hold an autonomous caseload, assess, progress, and discharge service users within agreed parameters.
* Collaborate with a multidisciplinary team, communicating effectively and organizing tasks independently.
* Provide support at other community hospital sites when required.
Qualifications
* HNC/HND in Health and Social Care or equivalent, or SCQF level 8 (e.g., Postural Stability Instructor).
* Experience working as a therapy support worker, preferably with older people.
* Ability to travel throughout the area (essential for this community post).

* Excellent organisational and communication skills.
* Ability to work independently and as part of a team.
* Willingness to work public holidays as required.
* Commitment to ongoing professional development (regular supervision, PDP, and training opportunities).
Working Conditions
Public holiday working is in operation. Bases are to be determined, covering throughout Fife. Flexibility to work across multiple locality sites is expected.
