About the Role
Discharge Coordinator – part of the Discharge Team within Calderdale and Huddersfield Foundation Trust (CHFT). The role is based across acute hospital wards and supports the implementation of the Home First Model.
Key Responsibilities
* Carry out assessments of patients to determine need for home support, equipment, and mobility.
* Develop and manage patient discharge plans, liaising with MDT members and relevant agencies.
* Co‑ordinate and facilitate timely hospital discharge and onward care arrangements.
* Maintain accurate and up‑to‑date information in electronic patient records (EPR).
* Support patients and carers with consent and information sharing.
* Provide hands‑on assessment and provision of low‑risk equipment such as walking aids and commodes.
* Work independently with allocated caseload, taking responsibility for patient progress.
* Escalate delays in patient pathways to ward managers or discharge supervisors.
* Assist with transport arrangements and hospital discharge clerical tasks.
* Collaborate with the MDT to complete discharge documentation and meet policy deadlines.
* Attend MDT, ward manager and Discharge Sister/Therapist meetings to review patients’ status.
Qualifications & Experience
• Formal qualifications and experience relevant to discharge planning and patient care (see person specification).
• Proven ability to work independently and manage caseloads under indirect supervision.
Working Conditions
The role requires working across hospital wards, community settings and occasional home visits. A 7‑day service schedule means availability for weekend and bank‑holiday shifts on a rota basis.
