What You'll Do
1. Craft Personalized Care Plans: Collaborate closely with residents, their families, and healthcare professionals to design and implement care plans tailored to individual needs.
2. Ensure Accuracy: Observe daily interactions and conditions, ensuring all care documentation is precise, legal, and clear for the team.
3. Lead with Confidence: Organize and guide your team at each shift, ensuring everyone understands their roles in providing excellent care.
4. Facilitate Seamless Transitions: Oversee shift handovers, ensuring effective communication and continuity of care.
5. Monitor Health: Perform routine observations and manage specific health needs with attention to detail and safety.
6. Administer Medications: Safely manage and administer medications according to MAR sheets and protocols.
7. Build Relationships: Foster trusting relationships with residents, families, and healthcare professionals, addressing concerns empathetically and promptly.
8. Promote Independence: Support residents in daily activities, encouraging autonomy while providing assistance as needed.
9. Act on Feedback: Respond to team input to prioritize resident health and well-being.
10. Champion Respect: Uphold residents' dignity and individuality, advocating for their needs and preferences.
11. Encourage Engagement: Facilitate social activities to enrich residents' lives and foster a vibrant community.
12. Prioritize Safety: Maintain vigilance on health and safety protocols to ensure a secure environment.
13. Support Nutrition: Advocate for balanced nutrition and hydration, monitoring residents at risk of weight loss or dehydration.
14. Respond to Emergencies: Act swiftly in emergency situations, utilizing the on-call system and reflecting on practices to improve care.
About You
* You’re a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC PIN.
* Care home experience is a plus, but your passion for caring and dedication to residents’ lives are what truly matter.
* You connect naturally with residents, families, and colleagues, bringing warmth, understanding, and patience to every interaction.
* Excellent communication skills are essential—whether discussing needs with residents, updating families, or guiding your team, you do so with clarity, empathy, and respect.
