Job Summary
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities
* In accordance with the practice timetable, as agreed, the post-holder will undertake duties including surgery consultations, telephone consultations, home visits, checking and signing repeat prescriptions, and managing queries, paperwork, and correspondence promptly.
* Make autonomous, professional decisions regarding presenting problems, whether self-referred or referred by other healthcare staff.
* Assess healthcare needs of patients with undifferentiated and undiagnosed issues.
* Screen patients for disease risk factors and early illness signs.
* Develop care plans in collaboration with patients, following current disease management protocols.
* Provide counseling and health education.
* Admit or discharge patients from the caseload and refer to other providers as appropriate.
* Record clear consultation notes to standard.
* Prepare and issue computer-generated prescriptions, avoiding handwritten ones.
* Prescribe according to the practice formulary or generically when appropriate.
Additional Responsibilities
Be aware of and comply with all relevant practice policies, including prescribing, confidentiality, data protection, and health & safety. Commit to lifelong learning, contribute to audits, and support the development of electronic patient records and coding. Attend training sessions as organized.

Handle sensitive patient and organizational information with confidentiality, sharing only with authorized persons in line with policies.
