Overview
12 month fixed term post to cover maternity leave. An opportunity has arisen within our Dunfermline and West Fife Hospital at Home service for an experienced Advanced Nurse Practitioner (ANP) or Nurse Practitioner (NP) to join the team for 12 months to support maternity leave cover. This role will support the Hospital at Home team to deliver safe, effective and person-centred care to patients within the primary care setting. The role will predominantly focus on the Urgent and Unscheduled Care needs of the older adult population and the complex treatment and management of acutely unwell, palliative or frail patients including: nursing and care home residents.
Responsibilities
* Deliver safe, effective, and person-centred care to patients in the community and primary care settings.
* Assess, diagnose, and manage urgent and unscheduled care needs in older adults, including complex treatment and palliative/frail cases.
* Collaborate with the wider Health and Social Care Team and work autonomously using clinical judgement and decision making.
* Travel across Fife to attend to acutely unwell patients in their homes or in community settings as required.
Qualifications and experience
* Advanced Nurse Practitioner: Postgraduate Diploma in Advanced Practice at Masters level; assessed as competent in this level of practice.
* Nurse Practitioner: Advanced clinical assessment and decision making module; ideally also Postgraduate Diploma in Advanced Practice (Level 11) and Non-Medical Prescribing.
* Excellent communication skills and ability to work as part of a wider Health and Social Care Team.

Secondment, eligibility and legal requirements
* Existing permanent NHS employees must discuss this opportunity with their substantive line manager. A secondment policy applies.
* External applicants with continuous NHS service must have a secondment agreement from their current board to apply for this fixed-term post.
* Right to work in the UK: NHS Scotland Boards will check entitlement to work. Sponsorship via UK Skilled Worker/Health & Care Worker Visa may be possible in certain circumstances, subject to sponsorship licencing and salary thresholds.
* From 1 April 2025, a different level of criminal records check may be required if the post is deemed a regulated role. If required, appointment will be subject to PVG scheme. Details will be confirmed by the Hiring Manager or Recruitment Team.
