Clinical Nurse Specialist Development Post

Is it your goal to become a Clinical Nurse Specialist in palliative care, but you havent got the required skills and experience to achieve it now? This is an opportunity for an exceptional nurse to join our community service.

As a senior nurse within the specialist community nursing team, you will be an integral part of the team and will be supported by the Advancing Practice CNS and wider MDT to develop your skills, competence, knowledge and experience to prepare you for a future career as a specialist palliative care CNS.


Main duties of the job

This is a rotationalCNS development post.

The post holder will provide senior nursing support topatients referred to Trinitys Community Services under the supervision of theClinical Nurse Specialists, whilst developing the competencies to become aClinical Nurse Specialist in Palliative Care.

The postholder will rotatebetween the three services which make up community services at Trinity,spending a minimum of 6 months in each:

* The specialist community nursing team, which includes the rapid response team;
* Thefirst contact team
* The Wandsworth End of Life Care Coordination Service.
* Undertake holistic assessments of patients, their families and or informal carers to identify their immediate and anticipated future palliative and end of life care needs.
* Under the supervision of a Clinical Nurse Specialist, provide advice and support to a caseload of patients; their carers; and other professionals involved in their care
* To act as a community nursing end of life champion, inspiring others providing care to patients referred by demonstrating a range of skills

Job responsibilities

SpecialistCommunity Nursing Team & Rapid Response Team

* Undertakeholistic assessments of patients, their families and or informal carers toidentify their immediate and anticipated future palliative and end of life careneeds
* Underthe supervision of a Clinical Nurse Specialist, provide advice and support to acaseload of patients; their carers; and other professionals involved in theircare and continually monitor and evaluate the effectiveness of anyinterventions, maintaining accurate patient records.
* Workcollaboratively and communicate effectively with other community health andsocial care agencies to support the coordination and management of palliativeand end of life care for patients referred to the service.
* Useassessments and interventions as opportunities to train, coach and mentor lessexperienced clinical staff and clinical visitors to the hospice

FirstContact Team

* To play a lead rolein receiving and managing referrals made to the hospice.
* To ensure a timely,responsive, and appropriate service for all key parties involved.
* Manage all aspectsof the referral system to the hospice, to include appropriateness, urgency, andlevel of priority.
* Undertake holistictelephone assessments of patients, their families and or informal carers toidentify their immediate and anticipated palliative care needs.
* Workcollaboratively with the community team and other community health and socialcare agencies, to support the coordination and management of patients referredto the hospice.

WandsworthEnd of Life Care Coordination Service (WEoLCCS)

* To act as a community nursing endof life champion, inspiring others providing care to patients referred by demonstratinga range of skills
* To coordinate and supervise servicesHealth & Personal Care Delivery Team (Royal Trinity Carers)
* To provide clinical support to theRoyal Trinity Carers and Care Coordinators
* To work closely with the communityspecialist palliative care team; continuing health care team and care agenciesto ensure care is planned to meet the unique and changing needs of thepatients, their families, and friends.
* To build and maintain professionalrelationships (internally and externally) and to promote Trinitys services


Person Specification


Qualifications

* Registered with the NMC
* Evidence of post registration continuing
* professional development relevant to the role, and
* willingness to undertake further study to meet the
* Community Nursing Experience


Experience

* Evidence of post registration experience within the field of palliative care and or end of life care
* Experience of training and developing others
* Experience of practicing at advanced clinical level with awareness of own limitations
* Experience of effective working across clinical, professional, and organisational boundaries
* Sound knowledge and understanding of the professional roles and contributions of all members of the multi-disciplinary team
* Commitment to and understanding of Clinical Governance
* Understanding of the organisation as a charity
