You’ll be providing assessments and support to older adults with mental health needs, working closely with families and the wider team to put the right care plans in place.

What we need from you:
* Social Work qualification
* Registration with Social Work England (SWE)
* Experience in CMHT and prefereblyolder persons’ mental health or related services
* Car Driver
