Onward Care Clinician

Dynamic and motivated Allied Health Professional or Registered Nurse looking for experience working in a Community Hospital. Full time Band 6 role on intermediate care wards in Tiverton Hospital supporting individuals during their inpatient stay and working with the wider multidisciplinary team to arrange timely, effective discharges.
Job Responsibilities
* Lead admissions and discharges to and from Tiverton Community Inpatient Ward.
* Work with the multidisciplinary team to manage timely and effective discharge plans and ensure onward referrals are triaged appropriately.
* Assist the Clinical Nurse Manager and Clinical Therapy Lead with day‑to‑day bed management of the ward.
* Supervise and develop Band 5 nurses/therapists and unregistered nursing and therapy support workers.
* Use IT and communication skills; work independently and as part of the team.
* Manage a wide and varied caseload, including patients with long‑term neurological and orthopedic conditions, dementia, and rehabilitative needs.
* Encourage integrated working with NHS colleagues in multidisciplinary teams and with DCC, social care, and third‑sector partners to deliver a high‑quality service.
* Participate in professional development, in‑house training, supervision and external learning.
Working Pattern
37.5 hours per week, Monday‑Friday 08:00‑16:00.
