Main duties of the job
Join us in this innovative role where you'll support people to remain in their own homes rather than attending hospital, or facilitate rapid discharge when appropriate, using the Hospital@Home model. You will be an integral part of the Bromley Hospital@Home Service and the St Christophers Community Palliative Care Team, working closely with local acute and inpatient teams.

Job responsibilities
Position: Community Clinical Nurse Specialist (CNS) / Clinical Paramedic Specialist (CPS) (Hospital@Home)
Location: Bromley, with possible work from the Sydenham site
Hours: Part-time, 22.5 hours per week, fixed-term or secondment opportunity covering maternity leave
Salary: £32,250.50 - £36,390.50 (including HCA, for 22.5 hours/week). Salary will be determined based on experience and service.
We are looking for experienced community palliative care nurses or paramedics to join our team, supporting our innovative Hospital@Home service, which aims to prevent hospital admissions, enable early discharge, and support patients at home.
What will you be doing?
You will work across organizations to support individuals to stay at home or facilitate rapid discharge, supporting those reaching the end of life through the Palliative Pathway. You will work closely with the Bromley Hospital@Home Team, St Christophers Community Palliative Care Team, and other local health services.

Qualifications
* Essential: Proven palliative care/oncology experience with relevant qualifications, at Band 6 or above
Experience
Relevant experience in palliative care or oncology.
