Advanced Clinical Pharmacist - Haematology

Anexciting opportunity has arisen for a dedicated and motivated pharmacist tojoin the Cancer Services Pharmacy Team as an Advanced Clinical Pharmacist atLeeds Teaching Hospitals.

Thesuccessful post holder would provide an advanced pharmacy service to one ofour haematology disease groups that provide care to patients with severaldifferent haematological malignancies.

Weare looking for someone who is able to manage a dynamic and changing workloadproviding oversight of a specific sub speciality of Haematology. The postholder will be expected to utilise skills around medicines governance,finance and clinical trials.

Inaddition, the successful candidate would be responsible for the validation ofSystemic Anti-Cancer Therapy (SACT) and inputting to pharmaceutical care onour specialist inpatient wards.

Fulltraining would be provided to the new post holder as well as support toattain full competency in the validation of haematology related SACTutilising our comprehensive and structured training programme.


Main duties of the job

* Provide an advanced clinical pharmacy service to our inpatient wardsincluding those receiving inpatient Systemic Anti-Cancer Therapy (SACT)
* Validation of SACT across a range of sub specialities
* Attendance at speciality meetings and/or MDTs
* Support the implementation of new anti-cancer treatments within the postholders designated area of practice
* Support Drug & Therapeutics processes for individual and cohortapplications for the unlicensed use of medicines
* Undertake finance reporting to support the best value for medicinesagenda
* Undertake required audits and reviews to ensure services are adhering tolocal and national standards
* Provide training and supervision to colleagues in the cancer team and acrosspharmacy
* Supervise and line manage a junior pharmacist within the cancer servicespharmacy team

Job responsibilities


Clinical

Lead the highly specialisedclinical pharmacy service to patients and the multidisciplinary team within agiven clinical speciality.

Supports the Trust's PrescribingStandards and challenges practice where prescribers have not adhered to theMedicines Code.

Demonstrate the ability to providesafe, clinically effective and cost-effective use of medicines in accordancewith Trust policy, standard operating procedures and medicine legislation.This will include individual prescription review to optimise therapy,advising on dosage and side effects and on safe administration of medicines.The post holder is expected to determine how the outcomes are best achieve inspecific circumstances.

Pharmacists who are registered asprescribers will, with the agreement of the clinical director of thespeciality, prescribe within their level of competence, to support patientcare.

Plan, manage, monitor and reviewindividual patient care plans.

Participate in consultant wardrounds and multi-disciplinary team meetings.

Manage difficult and ambiguousproblems, often when information is limited.

Undertake risk management,including investigating clinical incidents in the clinical area and ensuringcompliance with medicine legislation.

The post holder will be expected toidentify, where necessary, areas that require service changes and develop a strategy to carry out the change through consultation with senior colleagueswithin and outside the pharmacy.

Manage the introduction of newdrugs, including unlicensed drugs, into the clinical speciality.

Manage the clinical pharmacistinput into clinical trials.

Develop and update policies for thesafe, effective and economic use of medicines through discussion with medicaland nursing staff.

Communication with Primary Careabout medicines policies or guidelines specific for their area of practice.


Management

Monitor and interpret expenditureon medicines within own clinical speciality. Provide the Directorate withregular reports to assist in the management of the drug budget

a. Attend the monthly ClinicalSpeciality Management meetings to advise on medicine use and financialimplications

- Horizon scan for new drugs andadvise the clinical speciality meetings on the financial implications

b. Co-ordinate Medicines Use SavingsProjects

c. Review prescribing data fromout-patient clinics

Contribution to financialinitiatives

Write or respond to business casesfor service development to secure additional funding.

Responsible for day to daysupervision / management of specialist clinical pharmacists in clinicalspeciality (some staff)


Researchand Education

Occasionally participates inresearch and development within clinical speciality

Undertakes own research/auditwithin clinical speciality

Supervise, teach, and assess juniorpharmacy staff completing training modules within the clinical area. Thisinput varies, mostly based on the experience of the post holder, and willtypically include:

- Acting as a trainer topre-registration and junior pharmacists in their area of specialist practice

- Being a named tutor for apre-registration and/or diploma pharmacist

- Lecturing on clinical issuesrelevant to the specialty, as part of the pre-registration training program,the clinical diploma, and any other in service training.

The post holder is also expected toparticipate in the training of other clinical professionals within theirspecialist area as requested.

Develop the role of a specialistpharmacist through national links.


Person Specification


Qualifications

* Registrant of the GPhC
* Postgraduate diploma in clinical pharmacy or equivalent
* Non medical prescriber status


Experience

* Understanding and appreciation of local and national pharmaceutical and clinical strategies
* Advanced clinical pharmacy knowledge in a relevant area
* Understanding of NHS/government policy and its applications
* Ability to demonstrate foundation level competencies in all 6 domains of the pharmacy framework
