About the Service -

The Urgent Community Response Service is a collaborative Health and Social Care initiative that supports individuals in their own homes during times of crisis. Our goal is to prevent unnecessary hospital admissions and enable timely discharges—helping people recover safely and comfortably at home.

Locations -

We’re expanding our team in the following bases:

* Crediton Community Hospital
* Okehampton Community Hospital.

Flexible Working Hours

* Service operates 7 days a week, between 07:30 and 22:00
* Full-time and part-time shifts available
* Enhanced rates for evenings and weekends

What does the job entail -

* Delivering high-quality care that promotes independence, dignity, and wellbeing
* Responding to urgent situations in collaboration with Community Nursing and Therapy Teams
* Supporting individuals to remain at home or return home swiftly from hospital
* Working independently in the community with ongoing support from managers and clinical teams

What We’re Looking For

* NVQ Level 3 in Health and Social Care
* Experience working with older people in a community setting
* IT literacy for electronic care recording
* A valid driving licence and access to your own vehicle (essential)

Lone Working with Strong Support

While our Support Workers operate independently in the community, you’ll never be alone. You’ll have access to:

* Clinical Team Lead
* Team Managers
* Community Nursing Teams

Our Healthcare Support Workers (HCSWs) form an essential and well-respected part of our healthcare teams across a number of dynamic settings; out in the community, in our acute hospitals or within our theatres teams to name just a few.

NVQ Level 3 in Health and Social Care is a desired criteria for this job. You will quickly become a valued member of the team through our comprehensive training and induction programme and our commitment to learning and development will satisfy even the most ambitious person. Once you’ve completed your training, and gained the necessary experience you will progress along the salary scale.

Working Pattern:There are two positions, one part time and one full time.Our service runs between 07:30am and 22:00pm. There is the option of AM or PM shifts.AM shift - 07:30am - 15:30pm or PM shift 14:30pm - 22:00pm.The service is a 7 day a week service. The ability to work weekends is essential.

Depending on your shift pattern and working hours, you may also receive additional enhancements to your pay.
