If so, we invite you to join our Tissue Viability Service as a Tissue Viability Support Nurse.
Job Summary
19.8 hours per week.
Responsibilities
* Support the coordination and delivery of specialist tissue viability advice and clinical expertise.
* Assist in the assessment, monitoring, and management of wounds of varying complexity.
* Contribute to high quality, evidence‑based wound management plans.
* Work collaboratively with nurses, AHPs, GPs, care providers and other professionals.
* Support continuity of care across community, clinic and ward settings.
* Promote best practice, safety and dignity in all aspects of care.
* Apply and develop additional skills linked to tissue viability.
* Contribute to a responsive, patient‑centred service that values holistic care.
* Provide support across patients' own homes, care and nursing homes, clinic and ward environments.
* Maintain accurate records, confidential and contemporaneous.
* Provide clinical supervision to other staff and students.
* Participate in team activities, professional development and governance.
* Advocate for patients and coordinate care across settings.
Key Duties
Work across multiple community settings, develop and apply specialist tissue viability skills, support CHC assessment, case management and patient journeys from referral to discharge.
Qualifications and Experience
* 1st level nurse with registration (degree or diploma).
* NMC Registration.
* Experience in a variety of healthcare settings.
* Knowledge of recent developments within health and social care.
* Ability to operate community equipment and identify urgent client information.
* Willingness to undertake further education around long‑term conditions and tissue viability.
* Evidence of continuing professional development.
* Experience of working in community nursing and caseload management (desirable).

Desirable Skills
* Advanced knowledge and competence in specialised skills: tissue viability, bladder & bowel, end of life, IV drug administration, diagnostic skills.
* Experience managing a caseload.
Person Specification
Team worker able to work alone, listen to and communicate with patients and carers compassionately, presentation skills or teaching to individuals, up to date knowledge of current practice, understanding of clinical supervision and CPD.
