The service is designed to provide an integrated model of social care that promotes a positive experience for service users and carers across the whole service. Lincolnshire Partnership NHS Foundation Trust (LPFT) has a Section 75 agreement under the National Health Service Act (2006) where Lincolnshire County Council has devolved their assessment and care management duties to LPFT. Social work in the Trust is delivered through a Community Social Care Resource where social care staff are aligned to the integrated community teams but are managed and supervised from within the social care resource. There are three distinct Social Work teams covering the East, West and South of the county respectively. Each team has a Team Manager, and managers are expected to work closely to provide a consistent service across the county. The post is permanent, part time, 22.5 hours per week, and suitable for Newly Qualified Social Workers (Band 5). The post is a key role in the Trust’s Community Social Care Resource, providing assessment and care management duties under the Care Act, supervising qualified and unqualified staff, contributing to community capacity building and supporting the Manager within the team. The base is flexible within the East area.
Responsibilities
 * Carry out direct work with service users and colleagues within other teams within the Trust; promoting multi-disciplinary working.
 * Provide Care Act assessments and Care Act reviews.
 * Completion of support planning.
 * Mental Capacity Assessments.
 * Undertaking duty responsibilities.
 * Multi-agency working within the wider Trust and outside agencies.
 * Following safeguarding principles.
