Early Diagnosis Cancer Navigator in Primary Care
We are excited to launch this innovative pilot role, led and funded by Lancashire & South Cumbria Cancer Alliance and hosted by Burnley Group Practice on behalf of Burnley West Primary Care Network. The role aims to improve early diagnosis of cancer.
This 12-month fixed-term post (employment or secondment) offers a unique opportunity to shape the delivery of support to your local population.
As an Early Diagnosis Cancer Navigator, you will support and develop coordination and management of early cancer detection and prevention across the Primary Care Network, ensuring the patient's cancer journey is as seamless as possible.

Main duties of the job
Patient experience of cancer care needs to be improved with better personalisation. The postholder will support and develop coordination and management of early detection and prevention of cancer across the PCN, ensuring a seamless patient journey.
This includes assisting Burnley West PCN in delivering improvements related to national cancer screening programmes.
The postholder will develop supportive relationships with Practice staff to ensure robust referral practices, including guidelines, professional development, and safety netting for suspected cancer cases.

Burnley West PCN brings together 5 local GP Practices to provide effective and positive care, prioritising patient-centred healthcare by collaborating with community, mental health, social care, pharmacies, hospitals, and voluntary services.
Job responsibilities
PRIMARY DUTIES AND RESPONSIBILITIES
* Assist Burnley West PCN in improving services, including participation in national cancer screening programmes.
* Ensure supportive referral practices for suspected cancer cases, using guidelines, professional development, and safety netting.
* Build supportive relationships with practice staff, including non-clinical cancer champions, social prescribers, pharmacists, and secretaries.
* Evaluate screening uptake, engage hard-to-reach populations, and reduce health inequalities by enhancing tracking and follow-up processes.
* Monitor patients suspected or confirmed to have cancer, ensuring timely processing according to Cancer Waiting Time Targets.
* Advise practices on cancer audits and referral reviews, and collate learning from case reviews.
Support the development of key workstreams to improve screening coverage and facilitate timely diagnosis, including maintaining a cancer database for tracking patient care and progression.
* Monitor and support patients post-treatment, ensuring follow-up at 3 and 12 months as per the Quality & Outcomes Framework.
* Identify and resolve delays in the patient pathway, discussing solutions with practices or the PCN.
* Develop consistent safety netting approaches for urgent referrals.
* Support peer learning events, sourcing resources from organizations like Cancer Alliance, Cancer Research UK, Macmillan, and local authorities.
* Assist practices in developing and implementing cancer screening improvement plans.
* Create resources to support patient information in various formats, including for those with learning disabilities or non-English speakers.
* Review practice coding for report building, ensuring data accuracy and consistency across the PCN.
* Provide administrative support for project evaluation and feedback.
Person Specification
Skills and Knowledge
* Excellent communication skills with healthcare professionals and diverse patient groups.
* Ability to establish effective working relationships within the PCN and wider health community.
* Knowledge of cancer screening programmes, particularly bowel, breast, and cervical cancer.
* IT literacy with data analysis skills and experience with patient information systems.
* Previous NHS or similar administrative experience.
* Knowledge of medical terminology, anatomy, and physiology.
* Organizational skills and ability to work under tight deadlines.
