AS THE UTILIZATION REVIEW - REGISTERED NURSE, you'll be responsible for the comprehensive, interdisciplinary coordination of patient care to ensure that quality care and patient outcomes are achieved within established time frames. This individual performs comprehensive patient assessments and interviews, resource management, utilization management, discharge planning, and facilitates referrals to other levels of care including the swing bed program and outside facility referrals. They will work collaboratively with the multidisciplinary care team to facilitate the achievement of desired treatment outcomes to assure access to appropriate levels of service within the hospital and efficient use of economic, technologic, and human resources throughout the patient's hospitalization. The position will implement coordinated, interdisciplinary plans of care; serve as a clinical resource in identified specialty areas; work closely with the medical staff to develop clinical order sets; serve as a patient advocate; analyze data and develop strategies to improve care delivery and function as an integral part of the care delivery team.

Qualifications


* Registered Nursing License in the state of Pennsylvania required
* Bachelor's degree in Nursing preferred
* Two (2) years of clinical experience required, with preference for acute experience
* Case management certification preferred
* Case management / utilization review experience preferred
* Literate in software and information systems
