We’re a community-based therapy service where you’ll prevent unnecessary hospital admissions and improve quality of life for service-users while rehabilitating in a home environment.
We offer specialist development, multidisciplinary working and key leadership skill development.
About the Service
The Sirona Integrated Neighbourhood Teams (INT) provide active rehabilitation in patients’ homes to maximise functional potential and support independent living.
You will work in a friendly and supportive INT alongside Therapists, Assistant Practitioners, Nurses, Nurse Associates, Advanced Clinical Practitioners, Support Workers and Administrators.
Flexible Working
We support flexible patterns – this service runs 8am‑8pm, 7 days a week.
Enhanced pay for weekends & bank holidays, mileage reimbursed during duties at work.
Responsibilities
* Initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell, often with complex needs.
* Use clinical reasoning to assess, plan, implement and evaluate patient‑centred intervention programmes using evidence‑based practice, promoting high quality and harm‑free care.
* Responsible for proactive service and own caseload management.
* Responsible for allocation of work and efficient service/team coordination.
* Develop and maintain strong, communicative relationships to prevent avoidable admissions and facilitate timely discharges.
* Collaborate and negotiate with service users, carers and family to identify achievable goals for the overall care plan and maximise quality of life and independence.
* Facilitate appropriate referrals to other services based on assessment.
* Assess and arrange provision of equipment within scope of practice; report any issues.
* Take responsibility for developing other INT members – peers, junior staff, and students.
* Maintain accurate, informative and legal records.
* Actively participate in training; may assist in delivery.
* Identify risks and act to minimise risk to self, colleagues and service users.
