Care Coordinator - CPN at Greater Manchester Mental Health NHS Foundation Trust
We are looking for a enthusiastic Community Psychiatric Nurse to join our friendly team at Trafford North CMHT. This wide‑ranging role offers a wealth of learning opportunities. The desired candidate will have proven experience working in a demanding mental health setting and awareness of delivering community mental health care. You will hold and manage a caseload of service users all with severe mental illness (SMI) and have Care Coordinating responsibilities under the Care Program Approach Guidelines. You will have proven experience of integrated working with inpatient units, HBTT, social care sector, police and third sector providers. A knowledge of the implementation of the MHA within community settings would be beneficial, along with an understanding of provision of care packages and associated documentation. The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds, and with a wide range of psychosocial needs. By assessing your clients and their carer's needs you will be able to access and sign‑post to a wide range of community resources and provider services available in the local area.

Responsibilities
* Assessments, care planning, psychosocial interventions, safeguarding, duty assessments.
* Report writing, attending tribunal hearings, supporting with the depot clinic.
