Job overview

This is a valuable opportunity for those wishing to broaden their experience or pursue a career in hospital medicine. The post is based at the Imperial College Renal and Transplant Centre at Hammersmith Hospital, which is the largest renal and transplant center in Europe. We cover kidney care across north-west London, and have extensive experience in managing patients with complex glomerulonephritis, renal transplantation and dialysis.

Successful applicants will join a rota of IMT and foundation doctors. Inpatient clinical duties involve rotating through day and night shifts covering our renal high dependency unit (with acute renal transplantation), three acute nephrology wards, our renal admissions unit and acute dialysis unit. We work closely with our surgical colleagues in renal transplantation and vascular access. The rota also provides significant exposure to renal outpatient clinics including general nephrology, low clearance, transplant, lupus, and vasculitis. The post-holder will acquire broad experience of specialist renal medicine, and also of general / acute medical problems occurring in patients with renal disease. All clinical work is under consultant supervision.

Main duties of the job

Ward work (this includes all jobs generated on ward round including TTOs, blood forms, clerking new patients, referrals, following up patients already discharged, assessing acutely ill patients, MDT meetings). Includes renal high dependency unit with level 2 care and also acute transplantation. Also 3 acute nephrology wards.

The outpatient clinic is a great opportunity not offered in many other placements. The FY2 will see patients with chronic renal disease and transplant patients. Very different treatment and management to one on the wards. Main jobs are assessing volume status, quick through histories and follow-up of patients. Clinics may be general nephrology (at satellite units across NW London) or specialty clinics (on site Hammersmith).

Admissions Unit Cover 1500 - 2100. Seeing patients in the ambulatory RAU or the monitored RHTU who may be admitted.

Auchi dialysis unit cover 1000 – 1600.

Working for our organisation

We are an academic unit closely integrated with Imperial College London, including the medical school. There is great opportunity to participate in audit, quality improvement, research and teaching. We run several national postgraduate courses in renal histopathology, glomerulonephritis, renal transplantation, dialysis, and nephrology for the acute physician. Study leave is encouraged, and a study budget will be provided.

Outline of timetable

Typical working pattern in the placement:

Ward/HDU Long days: 0815 – 2115

Nights: 2015 – 0915

Clinics/dialysis/admissions 6-hour shifts.

On call requirements: 2.5 weeks of sets of nights (each 13 hours shifts) in 4 month rotation

0820 Lupus Hot Cases MDT (weekly)

0915 Renal Histopathology meeting (weekly)

1130 Local renal teaching for FY/IMT (weekly). This time is protected for FY/IMT doctors but many SpRs, particularly new junior SpRs, join if they are free to do so and find this beneficial.

1230 Hammersmith Hospital grand round (weekly)

1330 Renal grand round (monthly)

1330 Vasculitis and GN Hot Cases MDT (weekly)

1400 Renal open meeting (Multidisciplinary, monthly)

Wednesday/Thursday

1500 Complex Transplant Hot Cases MDT (FORTNIGHTLY)

Friday

1200 Renal FY/IMT Peer Teaching Session (fortnightly). This is a teaching session lead by FY/IMT doctors for their peers, but is facilitated by an SpR or a consultant. It is a great opportunity for an SpR to chair a session and then undertake a teaching observation on the FY/IMT doctors, therefore expanding your skills as a trainer/educator.

Essential criteria

1. MBBS or equivalent medical qualification.
2. Full GMC Registration with a license to practice.
3. Can demonstrate the skills and knowledge, equivalent to the completion of Foundation level training or equivalent by commencement of post

Clinical skills

4. Demonstrates ability to carry out clinical skills appropriate to job description.
5. Experience of/aptitude for clinical activity appropriate to the role. At least 12 months’ relevant experience following award of medical qualification.

Desirable criteria

6. Experience of teaching junior medical colleagues and/or undergraduates

7. Able to communicate in spoken and written English to the standard necessary to fulfil the job requirements.

Audit

8. Able to demonstrate an understanding of the principles of clinical audit, clinical quality improvement initiatives and research including an awareness of ethics.
