The Community Services Coordinator is responsible for the smooth day to day running of the community services, supports the Homecare and Home Service Support Workers and ensures the service is delivered to a consistently high standard in accordance with Age UK Doncaster policy framework, mission and values.

Driving is essential to the role.
35 hours per week – Monday – Friday 8:30am-4:30pm. Flexibility is required as the post includes evening and weekend working. On call duties are paid in addition to salary.
Main duties for the role:
1. To take part in the recruitment and selection of community service workers in line with Age UK Doncaster's Recruitment and Selection Policy
2. To provide support to the community service workers through a variety of methods including regular supervision, team meetings and annual appraisal in line with Age UK Doncaster's Supervision and Performance Management Policy
3. To ensure all compliments, complaints, incidents, accidents and concerns are dealt with promptly in accordance with Age UK Doncaster policy framework.
4. To permanently allocate care packages to ensure consistent weekly rota's for service users and staff
5. To allocate annual leave for community service workers and organise appropriate cover for annual leave and any short notice absence.
6. To maintain client and staff records confidentially in line with Age UK Doncaster's policy and ensure that data it is appropriately recorded, stored and transferred
7. To deal with general enquiries and liaise with service users, their families and other professionals where appropriate.
8. To carry out client assessments and risk assessments, reviews and formulate care and support plans where required.
9. To work closely with the wider team to always ensure appropriate cover for all Services as per Age UK Doncaster Continuity Plan.
10. To liaise with statutory agencies as required and at all times maintain professional relationships
11. To take part in the on-call Rota. This will include alternate weekend working.
12. To carry out the direct provision of care where required.
13. To undertake any other reasonable duties in line with the job purpose that may from time to time be required by management.
