We are looking for a motivated Pharmacy Technician to join our forward-thinking, dynamic and supportive practice team.

You will play a key role in supporting safe, effective, and efficient medicines use across the practice. Working alongside our Clinical Pharmacists, GPs and wider multidisciplinary team you will help deliver high-quality medicines management and optimisation to improve patient care and ensure best use of NHS resources.

Main duties of the job

The Pharmacy Technician will support safe, effective, and efficient medicines use across the practice. Duties include managing repeat prescriptions, conducting basic medication reviews under pharmacist supervision, and supporting medicines optimisation and formulary compliance. You will perform medication switches or adjustments, assist with patient and prescriber queries, and co-ordinate medication reviews.

You will undertake medicines reconciliation after hospital discharge, liaise with community pharmacies and hospitals, and ensure accurate record keeping.

The role also involves supporting MHRA alerts, audits, and safety initiatives, providing patient education on medicines use, and contributing to quality frameworks such as QOF and IIF. You will work collaboratively within the multidisciplinary team to enhance prescribing safety and patient care.

As part of the Hounslow Health Primary Care Network, we collaborate closely with partner practices to deliver innovative and integrated healthcare.

We currently serve a list of over 15,000 patients with a team that includes GPs, nurses, pharmacists, healthcare assistants, physician assistants, first contact physiotherapists, social care prescribers and wellbeing coaches. Job responsibilities

Main duties of the job

Support and manage repeat prescription processes, ensuring accuracy, safety, and cost-effectiveness.

Conduct basic medication reviews under the supervision of a Clinical Pharmacist.

Support medicines optimisation activities and implement formulary or prescribing policies.

Perform medication switches or dose adjustments as directed by prescribers or pharmacists.

Assist with medication queries from patients, care homes, community pharmacies, and prescribers, and forward recommendations for alternatives.

Co-ordinate medication review appointments and any changes of medication as advised by the clinical team.

Undertake medicines reconciliation for patients after hospital discharge or transfer of care.

Liaise with community pharmacies, hospitals, and other care providers to ensure continuity of care.

Update electronic health records accurately and promptly.

3. Safety, Quality, and Compliance

Support implementation of MHRA alerts, safety updates, and prescribing audits.

Participate in audits, incident reporting, and continuous quality improvement initiatives.

Utilise the GP practice clinical systems (SystmOne) to conduct prescribing audits.

Participate in practice and multidisciplinary meetings to promote safe, effective prescribing.

4. Patient Support and Communication

Provide education and advice to patients about safe and effective use of medicines (within scope of practice).

Signpost patients to appropriate services or self-care options where suitable.

Support initiatives to improve patient understanding and adherence to prescribed treatments.

5. Practice Development

Contribute to the development and improvement of medicines management systems within the practice.

Support QOF, IIF, and other contractual or quality frameworks related to prescribing.

Participate in multidisciplinary team meetings and share learning with colleagues.


Person Specification


Qualifications

* Qualified Pharmacy Technician registered with the General Pharmaceutical Council (GPhC).
* Evidence of continuing professional development (CPD) relevant to pharmacy practice.
* Good general education, including GCSE (or equivalent) in English and Maths.
* Experience in a GP practice or PCN environment
* Completion of or working towards a Primary Care or Medicines Optimisation qualification (e.g. CPPE Primary Care Pharmacy Education Pathway).
* Experience or training in clinical audit, prescribing safety, or medicines management, medicines reconciliation or repeat prescribing audits
* Knowledge of SystmOne or similar GP clinical systems.
* Additional training in high-risk drug monitoring, repeat prescribing or quality improvement initiatives.
