We have a vacancy in the Adult Hospital First Contact Team. The role also includes an element of Out of Hours shifts.
Ideally candidates will have experience of crisis management and multidisciplinary work. The role incorporates both discharge planning for patients in Sheffield Teaching Hospitals, and the team covers a citywide response evenings and weekends, including hospitals. Relevant IT skills will be required but additional training will be provided for specific systems.
Assessment and duty experience would be an advantage, but a flexible approach is important. Safeguarding experience is essential. The ability to work to short time scales and problem solve is also essential.
The team is based at the Northern General Hospital and can include working with health colleagues across all disciplines. Close working with the Short Term Intervention Team and other council services is also part of the role. The role is very varied, no two shifts are the same.

You should hold registration with Social Work England.
