We have a new and exciting opportunity for the right candidate to join Swansea Bay University Health Board's Primary Care and Therapies Group.

The successful applicant will provide professional, holistic nursing care and support to patients, carers and families within a variety of community settings, delivered with compassion, dignity and respect.This post will require individuals that can deliver the highest quality care, ensuring evidence based/ best practice and has a commitment to continuous professional development.Applicants to have excellent communication skills and have a flexible and adaptable approach to changing work patterns. Variety of shift patterns over a 7 day service.We offer mentorship, working life balance and exciting professional development opportunities to meet your needs.


Main duties of the job

The successful candidates will be required to function as a member of the multi-disciplinary team, to assess individual patient's care needs and escalate concerns to the correct member of the team. Working as a lone worker responsible for carrying out the delegation of tasks, assessments and evaluations, and escalating concerns as appropriate.

This role involves working within the community, delivering high quality care to a vast range of patients within their own home setting. There would be an expected allocation of work to be complete each shift, this includes visiting patients as a lone worker. The post will also involve transporting equipment such as medical stock and equipment to patients and to health board sites. Therefore, the ability to travel independently between sites in a timely manner is essential.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Person Specification


Qualifications & Knowledge

* Level 1 Registered Nurse (Adult)
* Current NMC Registration
* Evidence of personal continuing development, statutory and mandatory training
* Awareness of Health Board policies, procedures and guidelines
* Ability to relate theory to practice
* Understanding of primary health care working
* Post registration experience, in acute services.
* Teaching and assessing
* Understanding of clinical governance, health & safety, risk management awareness
* Community experience
* Willing to undertake further training as necessary.
* Evidence of post- registration education - Diploma / Degree Level


Experience

* Ability to demonstrate sound knowledge of NMC Code (2018)
* Able to relate theory to practice through reflective skills.
* Relevant community experience
* Ability to demonstrate sound knowledge of the community

£30,420 to £37,030 a yearper annum (pro rata if part time)
