An exciting opportunity for a Band 7 Community Team Leader has arisen for you join a dynamic, strategic, and enthusiastic Community Nursing Team in Wokingham. You will be joining a service that has been recognised as having an ‘Outstanding’ rating, with the trust also being rated ‘Outstanding’ in our latest CQC inspection.
In your role you will lead and support staff to promote a service which provides complex nursing care to patients in their own homes. You will be responsible for all aspects of the leadership, development, and management of designated teams.
Using your skills, you will ensure that safe, evidence-based care is delivered to patients, their families, and carers in the Community, whilst also being proactive in promoting a positive image of Community Nursing. You will be working closely with the organisation, integrated teams, key stakeholders, and partners.
Your Base to be confirm but will be expected to visit other sites as part of your role. The benefit of having core working days of Monday – Friday 08:30-16:30-one TeaTime 16:30-19:00 per month and one week-end in 7 weeks. but with opportunities for flexible working will enable you to ensure that a high-quality service is always delivered.
Main duties of the job
1. Responsible for the delivery and operational management of the service.
2. Directing and supporting staff in their clinical roles to promote a service which prevents inappropriate or unscheduled hospital admissions and facilitates timely discharge
3. Responsible for capacity planning and management of a seven-day service, liaising with other Team Leaders to
4. ensure equity of staffing and workload to meet the needs of the service
5. Your working hours would be mostly Mon to Fri, to (with approximately one weekend every 7 weeks and bank holidays in rotation). The Team Lead will also be covering the 4:30 to 19:00 pm shift on a Rota basis.

6. Adult nursing qualification along with NMC registration

9. A full UK Driving Licence and have use of a car for work purposes
