Band 6 Occupational Therapist – Community Care Coordinator Team
We have an exciting opportunity for a Band 6 Occupational Therapist in the position of Community Care Coordinator located in the vibrant area of Lewisham, UK. What you will do:
* Manage a caseload under the CPA, focusing on recovery-oriented care planning and risk management.
* Conduct home visits to assess and support client needs.
* Administer depot injections as necessary.
* Respond swiftly to urgent referrals, ensuring proper care and timely intervention.
* Liaise effectively with external agencies to facilitate comprehensive support for clients.
* Encourage and support independence and community integration for clients.
This position requires HCPC registration and a valid driving license as you will need to travel for home visits.
