Job description
Responsibilities include, but are not limited to:
* Conducting comprehensive assessments of individuals with learning disabilities to identify their strengths, needs, and preferences.
* Collaborating with individuals, families, and relevant stakeholders to develop person-centred support plans.
* Advocating for the rights and interests of individuals with learning disabilities within the community and various service settings.
* Providing emotional support and practical assistance to individuals and their families.
* Collaborating with multidisciplinary teams, including healthcare professionals, educators, and community organisations, to ensure coordinated and holistic support.
* Facilitating access to appropriate services, including healthcare, housing, education, and employment opportunities.
* Responding promptly to crisis situations and provide immediate support to individuals and families in distress.
* Developing crisis intervention plans and collaborate with relevant agencies to ensure ongoing safety and well-being.
* Maintaining accurate and up-to-date records of assessments, support plans, and interventions.
Requirements:
* Degree in Social Work or a related field.
* Registration with the Social Care Wales.
* Experience working with individuals with learning disabilities.
* Strong communication and interpersonal skills.
* Knowledge of relevant legislation and policies related to learning disabilities.
* Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
* Willingness to complete an enhanced DBS if you do not currently hold one.
